Please read the following in full prior to booking.

  1. All appointments are conducted via telehealth.

  2. A credit card is required to hold an appointment and is stored securely through my EMR. 

  3. After your appointment request has been approved, you must complete all forms in the client portal prior to the initial assessment. 

  4. Please provide at least 24 hours of notice to cancel or reschedule an appointment.

    • In the case of a no-show or late cancellation, your credit card will be charged for the appointment at my self-pay rate ($175/55 minutes), as this time is saved for you and I am unable to bill insurance for this time.

  5. I strongly recommend you verify your Medical Nutrition Therapy (MNT) benefits using this script.

    • You are not required to do this step, but know that you will be signing a legal document agreeing that if your insurance rejects the claim I submit for any reason, the credit card on file will be charged for the cost of the appointment at my self-pay rate ($175/55 minutes). If the card is denied, your bill will be sent to a collections agency.

    • If I am not in-network with your insurance plan, payment is due at the time of service. I can provide you with a document called a superbill (medical receipt). You can submit this to your insurance for reimbursement if you have out-of-network benefits.

    • For self-pay patients, payment is due at the time of service. For your convenience, I accept all credit cards, as well as HSA and FSA.
